Action Tracking & Management

Recommended actions from FMEA Optimization (Step 6) are assigned with a responsible person, target date, action type (prevention improvement, detection improvement, design change, process change), and priority derived from the AP level.

Actions progress through a defined lifecycle: **Open** → **In Progress** → **Completed** → **Verified Effective** → **Closed**. Each status change is timestamped and attributed. When marking an action complete, document what was done and provide evidence of implementation.

After an action is implemented, the FMEA team re-rates S/O/D for the affected failure chain. The system shows a before/after comparison — current state versus revised state — to verify measurable risk reduction. This closes the loop between risk identification and risk mitigation, transforming FMEA from a paperwork exercise into demonstrated continuous improvement.

**My Actions**: Your personal action list filtered by FMEA, due date, status, and AP level. **Organization Actions**: All open actions across all FMEAs with filters by responsible person, overdue status, and risk level. **FMEA Health**: Per-FMEA action completion rate, average days to close, and percentage of High AP items with completed actions.

Actions past their target date trigger email notifications. The dashboard shows aging: 0-30 days overdue (yellow), 30+ days overdue (red). Configurable escalation ensures overdue high-priority actions don’t fall through the cracks.
